Managing Chronic Health Conditions in Primary Care: How Can the Behavioral Health Consultant Help

About this session

Behavioral Health Consultants (BHC) are frequently sought to assist in the management of mental health conditions (e.g., depression, anxiety). However, the scope of a BHC in primary care goes beyond managing these and includes the management of chronic medical conditions such as diabetes, asthma, and chronic kidney disease. This session will highlight the evidence that supports the role of the BHC in the management of chronic medical conditions. By the end of the session, attendees will learn common interventions for the management of chronic medical conditions to feel better equipped to support primary care providers and patients in improving quality of life.

Yajaira Johnson-Esparza

Dr. Johnson-Esparza is a licensed clinical psychologist and Assistant Professor in the Department of Family and Community Medicine at UT Health San Antonio. She serves as Behavioral Health Consultant at Robert B. Green Family Health Center, a family medicine residency clinic. As Assistant Professor/Clinical and BHC, she provides integrated primary care behavioral health (PCBH) consultation services to a primary care population, as well as behavioral science education, training, and supervision of family medicine and psychology residents.
